Please complete the following form for pre-arrival treatment scheduling. We will immediately schedule these treatments as closely to your request as possible. Your treatment confirmation will be held at the Treatment Center desk until your arrival. At this time, our counseling staff will be available to assist you in determining if your pre-arrival schedule meets your goals and works with other activities you have planned.

*Due to problems associated with the time & travel zone changes, we do not preschedule on the day of arrival. Confirmation # or Reservation I.D. required. No reservations will be accepted until 2 weeks prior to arrival. Please do not make scheduling requests before this time.
